Cookie Policy
Effective Date: April 10, 2026 Last Updated: April 10, 2026
What Are Cookies?
Cookies are small text files placed on your device (computer, tablet, or phone) when you visit a website. They help the website recognize your device and remember information about your visit, such as your preferences and consent choices.
Similar technologies include pixels (tiny transparent images embedded in web pages), local storage (data stored in your browser), and scripts (code that runs in your browser to enable functionality).
How We Use Cookies
Vascular & Interventional Specialists (“VIS”) uses cookies and similar technologies on www.vascularcare.com to:
- Ensure the Website functions properly.
- Enable appointment request forms and call tracking.
- Analyze how visitors use the Website so we can improve it.
- Measure the effectiveness of our advertising campaigns.
- Deliver relevant advertising on third-party platforms.
Cookie Consent
When you first visit our Website, a consent banner powered by Complianz allows you to accept or decline non-essential cookies. You can change your preferences at any time by clicking the cookie settings link in the Website footer.
We use Google Consent Mode v2, which adjusts how Google tags behave based on your consent choices. If you decline analytics or marketing cookies, Google tags operate in a privacy-preserving mode that does not store cookies or collect identifiable data.
Strictly Necessary and Functional cookies cannot be disabled because they are essential for the Website to operate (e.g., displaying forms, routing phone calls).
Cookie Inventory
Strictly Necessary Cookies
These cookies are required for the Website to function. They cannot be disabled.
| Cookie | Provider | Purpose | Duration |

cmplz_* | Complianz | Stores your cookie consent preferences | 1 year |
__cf_bm | Cloudflare | Bot detection and security protection | 30 minutes |
__cfruid | Cloudflare | Rate limiting and DDoS protection | Session |
Functional Cookies
These cookies enable enhanced features and personalization. Disabling them may prevent appointment forms or call tracking from working properly.
| Cookie | Provider | Purpose | Duration |
|---|---|---|---|
calltrk_* | CallRail | Dynamic Number Insertion — displays trackable phone numbers based on your traffic source for marketing attribution | Session |
calltrk_referrer | CallRail | Stores the referring URL to attribute phone calls to marketing channels | 1 year |
calltrk_landing | CallRail | Stores your landing page URL for call attribution | 1 year |
calltrk_session_id | CallRail | Session identifier for call tracking | Session |
| (Jotform iframe) | Jotform | Jotform appointment request forms may set session cookies within the embedded iframe to maintain form state | Session |
About CallRail Dynamic Number Insertion (DNI): When you visit our Website, CallRail may swap the displayed phone number with a trackable number unique to your session. This allows us to understand which marketing channel (Google search, social media, direct visit, etc.) led you to call. All numbers connect to the same VIS offices — only the displayed number changes for attribution purposes.
Analytics Cookies
These cookies help us understand how visitors use the Website. They collect aggregated, statistical data. These cookies are only set if you consent to analytics cookies.

_ga | Google Analytics 4 | Distinguishes unique visitors by assigning a randomly generated ID | 2 years |
_ga_9HV5B2MYXJ | Google Analytics 4 | Maintains session state for our GA4 property | 2 years |
_gid | Google Analytics 4 | Distinguishes visitors within a 24-hour period | 24 hours |

Google Analytics 4 (GA4): We use GA4 to measure website traffic, page views, session duration, scroll depth, conversion events (form submissions, calls), and marketing channel performance. GA4 processes data on Google’s servers. IP addresses are anonymized. We do not enable Google’s advertising features within GA4 (no demographics or interest reporting).
Microsoft Bing UET: We use the Bing Universal Event Tag to track conversions from Bing advertising campaigns. It measures page visits and form submission events.
Marketing Cookies
These cookies are used for advertising conversion tracking and audience building. They are only set if you consent to marketing cookies.
| Cookie | Provider | Purpose | Duration |
|---|---|---|---|
_fbp | Meta Pixel | Identifies your browser for Facebook/Instagram ad delivery and conversion tracking | 3 months |

_gcl_au | Google Ads | Stores conversion data from Google Ads click identifiers | 3 months |
_gcl_aw | Google Ads | Stores Google Ads click information for conversion attribution | 3 months |
Meta Pixel: When you consent to marketing cookies, the Meta Pixel tracks page visits and conversion events (such as appointment form submissions) and sends this data to Meta (Facebook/Instagram). This allows us to measure ad effectiveness and build audiences for advertising. We also use the Meta Conversions API (server-side) for more reliable conversion tracking that is less affected by browser privacy restrictions.
Google Ads: Google Ads conversion tags track when a visitor who clicked on a Google ad completes a conversion action (appointment form submission or phone call). This data is used to measure campaign performance and optimize ad spend.
Third-Party Cookies and Tags Managed via GTM
All analytics and marketing tags on our Website are managed through Google Tag Manager (container ID: GTM-TZLF4G9L). GTM itself does not set cookies — it controls when and whether other tags fire based on your consent preferences as communicated through Complianz and Google Consent Mode v2.
Tag firing logic:
- No consent given: Only strictly necessary and functional tags fire.
- Analytics consent granted: GA4 and Bing UET tags fire.
- Marketing consent granted: Meta Pixel and Google Ads conversion tags fire.
